IML is a participating provider with UnitedHealthcare. Patients receive full in-network benefits when using IML with either their UnitedHealthcare Individual Health plan or a UnitedHealthcare Medicare Advantage Plan.
UnitedHealthcare has instituted a Laboratory Benefit Management Program for patients with commercial (non-Medicare) plans in Florida.
This Program manages lab test utilization for these patients through two separate processes.
BeaconLBS Advance Notification
UnitedHealthcare requires that physicians ordering certain tests, referred to as Decision Support Tests, provide Advance Notification through www.BeaconLBS.com prior to testing. The subsequent PDS Outcome Summary must be on file so that the test may be appropriately handled. This process is separate from the Pre-Authorization procedure required for some other health services.

Medical Policies
UnitedHealthcare has established Medical Policies that limit coverage of certain lab tests. Testing for patients whose diagnoses do not meet these guidelines will not be covered. In these cases, patients must be advised in advance of testing and indicate in writing that they are willing to accept financial responsibility for the charges in order for testing to be completed.
