Terms & Conditions must be accepted online to enter Gateway

In consideration for access to International Medical Laboratory, Inc. lab results through our Online Results Gateway, the User understands and agrees to the terms below.

These results and the related patient data constitute Electronic Protected Health Information (EPHI), which is protected by rules and regulations including under the Health Insurance Portability and Accountability Act ("HIPAA") that limits access to EPHI and prohibit its disclosure

1. You are aware of the federal and state prohibitions against improper access, use and disclosure of EPHI and you agree to abide by these rules and regulations and understand that your access, use and disclosure of EPHI may be reviewed periodically for such compliance.

2. You are an authorized user of the EPHI, and by logging in to view this EPHI you are agreeing to the these terms. If you do not agree to these terms you are not permitted to access this EPHI.

3. You are under a legal duty not to access, use or disclose the data except for treatment, payment, or operational purposes in accordance with the federal regulations.

4. You agree to comply with any policies and procedures issued regulating the use of this website access.

5. You agree to limit your use of the database to the minimum necessary information needed for your permissible use.

6. You agree to immediately report to International Medical Laboratory, Inc. any breach of confidentiality or misuse of the EPHI and related information.

7. In the event that you breach your duty of confidentiality, access information for other than a permissible use, or access more than the minimum necessary information, you acknowledge that you may be liable for civil and other penalties provided by law.

8. In the event that you breach your duty of confidentiality, access information for other than a permissible use, or access more than the necessary information, you agree to personally indemnify International Medical Laboratory, Inc. against any resulting civil liability, including penalties, fines, and attorneys’ fees that may be imposed upon IML either as a result of your breach or as a result of your negligent or reckless access to more than the minimum necessary information,

9. You are aware that you may be subject to criminal prosecution for intentionally disclosing or intentionally accessing more than the minimum necessary EPHI.

10. You are aware and acknowledge that IML has the right to revoke your right to use this EPHI database at any time for any reason.

11. You are aware and acknowledge that any misuse by you will result in the immediate revocation of your access in addition to all accounts associated with the office and/or practice in which you are employed and/or associated.

12. You are aware and agree to no longer access or use this EPHI and related information if told to do so by the office and/or practice in which you are employed or associated, and/or if your employment or contract with your office or practice terminates.
